Online access to your medical records
We are offering patients online access to their medical record. This allows you to make appointments, request repeat medications and access to view your medical records.
If you would like to take advantage of this service then please read the information provided below. Our Terms and Conditions explain fully the application process.
If you already use online access for making appointments and prescription requests, you will need to fill out the below application form if you wish to view further details of your medical record online. We need to have your written consent to allow you additional access to your medical records online.
Patients who do not already have online access will need to complete the below application form which is also available from our reception.
Photo Identification (ID):
When applying for online access to your medical records, you must bring to the practice (in person) the application form (link below) along with your photo ID. This can be a photo driving licence or passport. Each person must present their own form and proof of Identify.
Photo ID is necessary for security reasons to ensure that access to your records is only issued to you.
We will then set up your account and create a letter containing all your login details. This letter will be available to collect, in person, from the surgery. For security reasons we cannot give out login details by phone or email. Please allow 10 working days for it to be activated and we will contact you to advise when your login details are ready to be collected.
